ICOI Japanは,5月15-16日に横浜市のパシフィコ横浜で開かれた2010 ICOI JAPAN Implant Symposiumで,研究活動の活性化を目的に「優秀ポスター賞」を創設。会期中に発表されたポスター演題を審査委員が選考し,福岡歯科大学咬合修復学講座冠橋義歯学分野教授の佐藤博信氏と,延世大学(韓国)歯学部歯周組織再生研究所歯周病学のJoo-Yeon Sohn氏に授与しました。
また,次世代の歯科医療の担い手を育成する目的で設けた「優秀ポスター学生賞」は,昭和大学歯学部顎口腔疾患制御外科学教室の笠木星児氏に授与しました。ICOI Japanは,笠井氏を8月26日から独ハンブルクで開かれるICOI World Congress XXVIIに招待します。

歯槽骨造成術後の不成功例に対する臨床的検討

Information concerning the prognoses of remaining teeth in patients with implant supported prostheses is limited. The purpose of this study is to identify whether the different prognoses of remaining teeth between patients with implant supported prostheses and removable partial dentures. Forty nine partially edentulous patients with Eichner type B and C were randomly selected from the patients in the Department of Prostodontics and Oral Implantology, Fukuoka Dental College. They were treated by implant supported prostheses and removable partial denture from 1998 to 2006 and they have finished their three years follow-up. Twenty seven were treated by implant supported prostheses (Average 54 years old) and 22 were treated by removable partial dentures (Average 56 years old). Numbers of extracted teeth were compared in each other.
Man-Whitney Rank Sum Test was used as a statistical analysis. Numbers of extracted teeth are 0.3 in average among patients with implant supported prostheses and these are significantly lower than those (1.8 in average) of the patients with removable partial dentures. These results indicated an advantage of implant supported prostheses for partially edentulous patients and residual teeth might be protected by implant supported prostheses. However, since there is no consideration about periodontal condition of residual teeth, quality of the bone surrounded by implant and so many factors in this research, further survey should be necessary in future.

Various surgical techniques have been used to obtain adequate amounts of keratinized tissue around dental implants, including the free gingival graft, apically positioned flap, and acellular dermal matrix graft.; however, these techniques are usually performed before implant placement or at exposure of submerged implants, which can extend the healing period and lead patients to suffer from pain and discomfort through several operative stages. The aim of this case report is to describe a simultaneous interpositional FGG (iFGG) with placement of nonsubmerged implants in a patient lacking keratinized tissue and assess the long-term outcome of this grafted gingiva.
A 50-year-old female patient presented, complaining of partial edentulism in the right mandible. The posterior molar site of the edentulous mandible presented a thin band of keratinized tissue on the ridge crest and inadequate vestibular depth. The treatment plan included placement of three nonsubmerged implants in the positions of the mandibular right second premolar, first molar, and second molar. In addition, simultaneous free gingival grafting was planned on the molar area to increase the keratinized tissue. A wedge-shaped FGG, including epithelium-connective tissue (E-C) portion and connective tissue-only (CT) portion, was harvested from the palate. The CT portion was inserted under the buccal flap, and the E-C portion was tightly secured around the implants and to the lingual flap.
The gingival graft was firmly attached and was well maintained at 7-year follow-up, with no conspicuous shrinkage or reported discomfort during oral hygiene procedure. The use of simultaneous iFGG associated with placement of implants could minimize the number of surgical stages required and patient discomfort, while predictably providing an adequate zone of buccal keratinized tissue. Therefore, the technique could be considered as an alternative method in increasing the keratinized tissues for cases that have minimal amount of keratinized tissues.

【目的】近年,インプラント治療において歯槽骨の十分な幅や高さが不足した症例では,様々な歯槽骨造成術が行われており,良好な結果が得られている。しかし,歯槽骨造成術後に,感染などの不成功例も散見される。そこで今回われわれは昭和大学歯科病院口腔外科で行った歯槽骨造成術後のうち不成功例に対して臨床的に検討し,若干の知見を得たので報告する。
【対象および方法】2003年1月から2008年12月までの5年間に当科で歯槽骨造成術を施行した159症例中,失敗症例の13例(8.1%)を対象とし,手術部位,手術法,移植材料,不成功率の要因などについて検討した。
【結果】性別では男性7例,女性6例,平均年齢は51.7歳(29〜68歳)であった。不成功例の内訳は,インプラント脱落10例,骨吸収2例,移植骨の脱落が1例であった。造成部位は上顎前歯部4例,上顎臼歯部8例,下顎臼歯部1例であり,手術法では,GBRが5例,サイナスリフト4例,ベニアグラフト4例であった。移植骨の材料は下顎枝9例,オトガイ1例,人工骨単独2例,腸骨1例であった。GBR法で使用した遮断膜は,吸収性膜2,非吸収性膜1,チタンメッシュ1,その他1であった。骨造成全体における不成功例の原因では,造成骨自体の脱落や骨吸収は少なく,インプラントの骨結合が得られない症例が多く,GBRによる骨造成でその傾向が多くみられた。
【考察】歯槽骨造成後の不成功例については,インプラント脱落が多く,発症部位では上顎臼歯部が多かった。手術方法についてはGBRが多かった。以上のことから,骨造成単独よりも,GBRを併用したインプラント埋入においてより慎重に計画を立てることが重要と思われた。
